About the Primary Care Safety Net Assessment:

The Health Foundation for Western and Central New York hired John Snow, Inc. in May 2013 to complete comprehensive primary care assessments in Western and Central New York.

The purpose of the assessments are to provide information to the Health Foundation and primary care stakeholders throughout the regions on the capacity and strength of the existing primary care system and to provide recommendations on how the Health Foundation and the regions’ health care providers should move forward in order to take advantage of the dramatic changes that are occurring in our health care system.

Since May 2013, JSI has conducted primary care demand and capacity assessments, consumer surveys, an extensive series of interviews and site visits with primary care practices and other stakeholders, and environmental scans to understand the impact and implication of the Affordable Care Act.

JSI has also been working with North Shore Long Island Jewish Hospital’s Research Unit to compile a broad range of data on regional health status and primary care capacity.
